Reporting a Suspected Adverse Reaction or Adverse Event

Every report plays an important role in the continuous monitoring of the safe use of medicines. If you or someone in your care has experienced a suspected adverse reaction while using a medicine distributed by PharmaPool Kft., please report it through the NNGYK’s online adverse reaction reporting platform or directly to our company. You only need to submit the report through one of these channels.

If you submit your report to us, we may contact you for further information if necessary. Please therefore ensure that your contact details are accurate.

Please provide at least the following information:

Your name and contact details as the reporter, including your email address and telephone number
The patient’s details: initials, age or age group, body weight and sex
A description of the suspected adverse reaction, including the symptoms, date of onset and outcome
The name of the medicine used and, if known, its batch number and indication

Please do not provide any personal information beyond the details listed above.

Please describe the event in your own words. Include the patient’s initials, age or age group, sex and body weight; the symptoms experienced, when they began and their outcome; and the name, batch number and indication of the medicine used, if known. Please do not provide any unnecessary personal information.
